Important Update from Dr. Adedolapo Ibikunle and Shoreline Medical Society

After much consideration, I have made the difficult decision to leave my practice at Shoreline Medical Sidney, effective December 31, 2026.

It has been a privilege to serve as your family physician. I am deeply grateful for the trust you have placed in me and for the opportunity to participate in your care and that of your families. The relationships I have built with you, my patients, have been among the most rewarding aspects of my medical career. Unfortunately, I will not be transferring patients to another facility.

I understand that changes in healthcare providers can be unsettling and may be stressful. Please be assured that every effort is being made by Shoreline Medical to recruit another family physician or nurse practitioner to take over my practice while also identifying opportunities to transfer patients to other Shoreline providers. While these efforts are underway, I would encourage you to register with the Health Connect Registry.

If you have upcoming appointments, require prescription renewals, or have concerns about your ongoing care during this transition, please contact the clinic as soon as possible so that appropriate arrangements can be made.

Your medical records will remain securely stored at Shoreline Medical Sidney. You may request a copy of your medical records or that your records be transferred to a provider outside of Shoreline Medical by completing a medical records transfer request form at the clinic – please note that there is a fee associated with this. Any outstanding test results will come to Shoreline Medical Sidney and regardless of your attachment status, they will be followed up on.

Thank you for allowing me to be part of your healthcare journey. I wish you and your families continued health and well-being in the years ahead.

With sincere gratitude,

Dr. Adedolapo Ibikunle

Shoreline Medical Society is committed to making every reasonable effort to attach you to another family physician or nurse practitioner within the organization. In the interim, we encourage you to register with the Health Connect Registry. As opportunities arise for us to attach patients internally, we will take the approach supported by the College of Physicians and Surgeons of anonymous selection with family clustering. Patients will be contacted directly about this – please do not call the clinic. We will provide an update as things progress, and we thank you for your patience while our team works hard on this.
